That would have to the The Fly. I remember my brother and I watching it once because we were curious, and I vowed to never, ever watch it again. It was the worst movie I had seen and made me feel unwell.

The Human Centipede

I also attest that the Human Centipede is a very disturbing film to watch. The graphics are very unsettling and it's very hard to shake off. It's a very creepy film.

Clockwork Orange: It has a good subject, there is nothing too violent in the movie but the treatment of the story can make you sick.
Antichrist: The story and the graphic violence both are sickening.
